Abstract: At 8:45 a.m. on November 17, 2020, Employee #1 and a coworker, employed by an HVAC company, were working at a corrections center. They were upgrading the bolts on the metal panels that cover the ventilation duct system throughout the facility. The coworker had been working from an 8-foot A-frame ladder when Employee #1, who was the supervisor, had the coworker climb down and reposition the ladder. Employee #1 then climbed to the fifth step to show the coworker how to properly drill the hole and apply pressure on the drill. During this demonstration, Employee #1 lost balance and, as the ladder began to slide out from under him, he fell to the concrete floor, a fall height of 5 feet. Employee #1 sustained blunt force traumatic injuries to his head, back, and sternum, and was killed.
