Abstract: At 3:15 p.m. on October 5, 2020, an employee, a machine operator, was assigned by his supervisor to work on the Wysong hydraulic shear machine (model no. H52 and serial no. H52-478), so he could get experience working on new machines in the plant. A sheet of cardboard was fed through the machine. The employee had his hands in the point of operation, when he inadvertently activated the foot pedal control and the shear activated. The machine did not have any point of operation machine guarding. The employee suffered an amputation to his left pinky finger and was treated without hospitalization.
