Abstract: At 1:45 p.m. on August 21, 2020, Employee #1 climbed a ladder to the perlite mixer on the Deralite line. Employee #1 fell 9.5 feet off of the Perlite mixer catwalk, when the lid of the mixer he was using for stability came loose. Employee #1 sustained lacerations to his head and received 12 staples. Employee #1 was hospitalized.
