Abstract: At 11:00 a.m. on July 16, 2020, Employee #1, Employee #2, and Employee #3 were installing components on the sign, in an attempt to finish the sign upgrade, to be reinstalled on the pylon. The threaded bolt use in lieu of the manufactured required pin broke, causing the sign to fall onto the three employees. One employee was killed, and two employees were hospitalized for their injuries.
