Abstract: At 12:38 a.m. on July 8, 2020, an employee was troubleshooting an extrusion press tail stretcher with two coworkers when the light curtain tripped after something got on the sensor. The employee cleared the light curtain and reset it without locking out the machine. The employee started moving the metal toward the tail end with his hand resting on the stationary belt part of the table and his finger hanging over the edge. The employee caught his left pinky finger between the moving metal and the table and amputated his finger, requiring hospitalization.
