Abstract: At 8:45 a.m. on March 11, 2020, an employee had removed a tri-clover/tri-flow valve from the CIP line to repair. The employee had removed the valve and had changed out the O-rings and gasket and was in the process of putting back together the valve stem and valve body. The employee was holding the valve body in his hands, while a coworker applied air pressure to the valve stem to get the valve stem to sit inside the valve body. The air pressure was too strong and caused the valve stem to collapse into the valve body quickly, which pinched the employee's right pinky finger at the knuckle between the valve stem and valve body. The employee suffered a right pinky finger amputation and was treated without hospitalization.
