Abstract: At 3:38 p.m. on January 28, 2020, an employee was working as a temporary helper for a university logistics support service. The service operates warehouses and provides moving services in support of the university. The moving service involves dispatching employees with trucks to move furniture, appliances, and other items between office locations and the warehouse as needed. The employee and a coworker, who is a driver and regular employee, were dispatched to an office to remove a medical freezer and move it to another location. The employee was a new hire had had not received safety training after being assigned to work in the moving department. After they loaded the freezer onto the truck, the coworker directed the employee to stow the Waltco liftgate while he went to complete paperwork. While raising the liftgate, the employee had his left hand in the pinch point of the liftgate cam follower which closed and amputated his pinky and ring fingertips. The employee had not received training on operating the liftgate and was not warned about the liftgate pinch points. The employee required medical treatment without hospitalization.
