Abstract: At 3:45 p.m. on March 2, 2020, an employee working for a rotary actuator and automated valve manufacturer was performing a task known as "assembling an 8" on an automated butterfly valve. The employee was near the end of his work shift and indicated that he was trying to finish up one last valve before the end of the shift. The employee did not set the valve/actuator assembly on the testing arms before applying air pressure. Instead, he placed the assembly on the table on its side. The employee was attempting to stabilize the assembly with his right hand. When he applied air, he caught his right ring finger in the valve as it closed. The employee amputated his fingertip and was hospitalized.
