Abstract: At 8:00 a.m. on August 30, 2019, an employee, a maintenance mechanic, was working in the juice line palletizer area of a dried fruit packing and beverage production company. The machine was an Alvey model #881 automated palletizer used to pack and shrink-wrap bottled beverage products on wooden pallets. The employee was working below the vertical hoist elevator where the stack of bottled sweet tea products on a pallet were being lowered onto a conveyor. The employee was putting shims underneath the conveyor belt to elevate the pallet loading area to ensure that the pallets were on level with the rollers so that the products could be transported down the line. The machinery was not deenergized or locked out and tagged out prior to the employee attempting to adjust the conveyor level. As the employee reached into the lower corner of the conveyor, a pallet came down vertically and landed on his right ring finger. The employee was hospitalized to treat an amputation of the fingertip of his right ring finger.
