Abstract: At 7:00 a.m. on February 4, 2020, an employee and a coworker were tasked to perform preventive maintenance on supply fans 2 and 3. The workers switched off the fans but did not lock or tag the switch. The workers then proceeded to supply fan 2 and performed maintenance on the fan. The workers then moved to start working on supply fan 3. The coworker went and turned off the switch for supply fan 3 but again no lock or tag was applied to the switch. The employee was standing next to supply fan 3 observing the pulley slowing down on the fan motor. The employee touched he belt and his hand was pulled into the rotation and his finger was caught between the belt and the pulley. The employee suffered an amputation of his left middle fingertip and was treated without hospitalization.
