Abstract: At 9:30 a.m. on January 31, 2020, an employee was working in the storage yard, marking product size, type, etc. The employee was struck by falling bundles of steel box channel (48 feet by 4 inches by 10 inches, 1,564 pounds) that collapsed as he was stepping onto them to mark them. The bundles fell off the front of the dunnage that they had been placed on by another employee. The following are the factors that would have prevented the accident: proper and secure stacking of the steel bundles to prevent them from falling. The employee suffered multiple fractures to his leg, foot, heel, and ankle. The employee was transported to Riverside Community Hospital, where he was hospitalized for three days. Prior to and during the course of the Investigation, including, but not limited to February 5, 2020, the employer failed to ensure that stacked material (2 bundles of steel tube, 10x4x.375, 48 ft in length, approx. 2000 lbs) was secured in a way to prevent it from tipping, falling, collapsing, rolling, or spreading. As a result, on or about January 31, 2020, an employee fell from the bundle of steel that collapsed beneath him, trapping his leg, causing serious injuries. steel box channel (48 feet by 4 inches by 10 inches, 1564 lbs)
