Abstract: At 11:30 a.m. on December 3, 2019, Employee #1 was removing material from the granulator vacuum gate. Employee #1 placed his right hand inside the end of the vacuum to unclog the chute. Employee #1 sustained right hand index and middle fingertip amputations, and was hospitalized.
