Abstract: At 4:27 a.m. on September 27, 2019, an employee whose work involved material handling duties for a delivery company was operating a forklift while performing clean up tasks. The employee was circling a pole with the forklift when his left foot struck the pole. The employee fractured his left foot, requiring hospitalization and surgery.
