Abstract: At 8:30 a.m. on September 11, 2019, Employee #1 and a coworker were prepping to perform a blow down of a new fuel oil pipe system. The 2-inch pipe fittings and nipples came loose from the valve (130 psi) and hit Employee #1 in the head. The employee was hospitalized to treat a fractured skull and lacerations.
