Abstract: At 2:46 a.m. on August 10, 2019, Employee #1 was cleaning a perishable food manufacturing facility. He was hosing down the floor in the Formulation Room when he fell asleep while standing up. He fell forward, face down, onto the hard industrial floor. Employee #1 fractured his nose and skull and broke his teeth. He was hospitalized.
