Abstract: At 8:50 a.m. on July 26, 2019, Employee #1, electrician, was attempting to remove a bolt from a sump pit at a gas service station in order to repair the gas pump. Employee #1 was using a reciprocating (sawzall) saw attempting to cut the bolt off so he could get to the wires to make repairs. As he was sawing the metal bolt, there was a flash/flame of fire that started very quickly and was extinguished right away. Employee #1 received severe burns to several parts of the upper body. A coworker was standing at the sump pit when this fire occurred, and assisted Employee #1 out of the sump pit. Employee #1 was transported to the local hospital, where he was admitted to treat the burns.
