Abstract: At 6:30 a.m. on June 20, 2019, an employee was checking an exhaust fan belt tension for a condensation control system when a damper would not close all the way. The wind caused the fan blades to turn, which turned the sheaves on the pulleys holding the belt while the employee was holding onto the belt with his left hand. The employee suffered an amputation of his left index finger when it was caught between the pulley and the belt and was hospitalized.
