Abstract: At 11:12 a.m. on December 6, 2018, Employee #1 was washing the rear of a white Ford Explorer that was on the conveyor system in a car wash tunnel. A coworker drove a black Ford Expedition into the tunnel and guided it onto the conveyor belt at the entrance of the tunnel, approximately 20 feet away from Employee #1, who was washing the Ford Explorer. The coworker attempted to put the black Ford Expedition in neutral and accidentally accelerated, striking the white Ford Explorer and pinning Employee #1 between the two cars. Employee #1 was hospitalized and sustained an amputation of his right leg.
