Abstract: At 1:00 p.m. on May 24, 2018, an employee was assisting driver unhitch a container from a truck. The driver thought that the container was unhitched and the employee was out of the way so he proceeded to move his truck forward. That was not the case and the container was still attached to the truck when the driver moved the truck forward. When the truck moved, it dragged the container which went over employee's left foot, causing an amputation to his middle toe.
