Abstract: At 6:15 p.m. on October 8, 2018, an employee at an oil well was tied off with fall protection on a pumpjack and was guiding the pump jack head into place. The pump jack head was connected by a chain and a steel cable. The employee grabbed the pump jack causing slacking in the line. The employee's arm became pinched between the pump jack head and the pump jack connection, resulting in an amputation at the forearm.
