Abstract: At 7:30 p.m. on April 2, 2018, an employee was working for a firm engaged in millwork, including flooring. He was at the facility's Work Center Number 1201. He was trying to undo a jam on a finger joint machine. He did not have the machine locked out or tagged out. When the jammed part had been dislodged, something crossed over the "cut eye." The cut eye sensed movement. At that point, the carriage opened. As the carriage opened, the ring finger on the employee's left hand became pinched. The employee sustained the amputation of his fingertip just below the finger nail. The narrative did not say whether he was hospitalized.
