Abstract: At 8:13 a.m. on February 13, 2018, an employee and coworker were removing two 3 foot by 6 foot metal grates inside of a decommissioned sub pump station to allow for old equipment to be hoisted from a 35 foot dry well. After removing the first grate and carrying the second grate near the first grate hole, the employee fell down the dry well and was killed.
