Abstract: At 12:30 p.m. on November 8, 2017, an employee was reaching down into a septic tank approximately 3 to 4 feet to grab an alarm unit in order to replace it. The employee fell into the septic tank, head first. Once extricated, it was learned that the employee was also submerged in septic water from his head and just past the shoulders. Findings indicate the employee died due to asphyxiation and/or suffocation.
