Abstract: At 10:30 a.m. on October 13, 2017, an employee was assisting a spot-welder operator by holding the back end of a work piece. Nuts were being welded onto the work piece. The employee reached past the welder operator to re-position a misplaced nut before it was welded into the wrong position. The welder is operated by a two-handed trip switch. As the assistant reached passed the operator, the welding cycle was initiated by the operator. The assistant's right index finger was caught between the top electrode and the work piece. The employee's right index finger was amputated at the second knuckle.
